Speech-Language Pathologist (SLP) - Home Health | $140/

$140 per visit

Per Diem / Contract We are a home health care agency seeking a licensed Speech-Language Pathologist (SLP) to provide an initial evaluation and follow-up speech therapy visits for a patient in Fullerton, Orange County . The frequency of visits will be determined by the clinician based on the patient's clinical needs and professional expertise. We strongly prefer a clinician who can provide continuity of care by completing the initial evaluation and continuing with follow-up visits through discharge . Responsibilities Perform an initial comprehensive speech and language evaluation. Develop an appropriate plan of care based on the patient's needs. Provide follow-up therapy visits as clinically indicated. Monitor and document the patient's progress. Coordinate care with the patient, family/caregivers, and interdisciplinary home health team. Complete all required clinical documentation in a timely manner. Provide care through discharge whenever possible. Qualifications Current California Speech-Language Pathologist license. ASHA certification (CCC-SLP) preferred. Home health experience preferred, but qualified clinicians with other relevant experience are welcome to apply. Reliable transportation and ability to travel to the patient's home in Fullerton. Strong clinical, communication, and documentation skills. Compensation $140 per completed visit.
