Responsibilities of a home-based Speech Therapist include : Provide high quality evaluation and treatment of disorders related to speech and swallowing Develop personalized care plans to improve one's ability to vocalize, communicate, eat, and overall function of patients in the home setting, involving them in setting goals Communicate and collaborate with associated medical professionals to achieve patient goals Clear, concise, timely documentation in our software system Regular communication with physicians, nurses, case managers within Veterans Affairs as needed Ability to work independently and autonomously, managing one's own schedule Supervise home health aides involved in patient care Benefits of working at Right at Home for a home-based
