Job Description

Park River Healthcare and Rehabilitation Center is seeking a dedicated and compassionate Speech Therapist (Speech-Language Pathologist) to join our rehabilitation team. This full-time position will primarily be based at Park River Healthcare and Rehabilitation Center, with two days per week providing services at New Brighton Care Center . This is an excellent opportunity to work with a collaborative interdisciplinary team in a long-term care and rehabilitation setting. Position Summary The Speech Therapist evaluates, diagnoses, and treats residents with speech, language, cognitive, communication, and swallowing disorders. The therapist will develop individualized treatment plans to help residents improve their communication abilities, cognitive functioning, and quality of life. Responsibilities Evaluate residents for speech, language, cognitive, voice, and swallowing disorders Develop and implement individualized treatment plans based on resident needs Provide therapy services in accordance with physician orders and care plans Document evaluations, treatment plans, and progress notes in the electronic medical record Collaborate with physicians, nursing staff, therapy team members, and families regarding resident progress Participate in care plan meetings and interdisciplinary team meetings Educate residents, families, and staff on therapy techniques and safe swallowing practices Maintain compliance with state, federal, and facility regulations Qualifications Master's Degree in Speech-Language Pathology Current Speech-Language Pathologist (SLP) license in the applicable state Certificate of Clinical Competence (CCC-SLP) preferred or Clinical Fellowship candidates welcome Experience in long-term care, skilled nursing, or rehabilitation preferred Strong communication, documentation, and interpersonal skills Ability to travel between facilities as required Benefits Competitive salary Comprehensive benefits package Paid time off Professional development opportunities Supportive interdisciplinary team environment
Pay:
$45.00 - $52.00 per hour
