Speech Therapist (SLP) - Per-Diem/PRN Northeast Professional Home Care is currently seeking an experienced Speech therapist for a per-diem opportunity performing home health visits for Stark and Summit counties. This position will be responsible for initial assessment of patient needs, developing plan of care, and providing necessary treatment. Paid Per Visit with a Flexible Schedule.

Qualifications:

Masters in Speech Language Pathology from an accredited program Current Speech Language Pathology license in the state of Ohio Current CPR certification required Customer service and communication skills desired Responsibilities Performs Comprehensive evaluation of the patient's level of function and participation in the development of the total patient care plan. Plans and provides rehabilitation services for patients with loss, impairment of abilities to complete activities of daily living Performs direct speech therapy treatments according to the physicians plan of treatment Establishes and updates the speech therapy plan of treatment. Reports changes in patient condition to appropriate personnel and the attending physician. Provides patient care education to clients and other caregivers. Participates in coordination of services with other disciplines and community resources as needed.
