Full Time Summary:
The Case Manager for High-Fidelity Wraparound provides intensive care coordination and case management services to children and adolescents with complex behavioral health needs and their families. Using a high-fidelity, wraparound-informed approach, the case manager partners with youth, caregivers, natural supports, and providers to develop and implement individualized, strength-based plans that promote safety, stability, and long-term success in the community.ESSENTIAL FUNCTIONS
Establish and maintain strong, trusting partnerships with youth and caregivers using family-driven, youth-guided principles. Engage youth and families as equal decision-makers in all aspects of planning and service coordination. Identify and build on youth, family, and community strengths. Facilitate Child and Family Team (CFT) meetings following high-fidelity wraparound standards. Develop, implement, and regularly update individualized wraparound plans of care that are strengths-based, culturally responsive, and outcomes-focused with youth and family voice and choice including natural supports and shared responsibility across team members. Coordinate services across multiple systems, including behavioral health, education, child welfare, juvenile justice, developmental services, and medical providers. Serve as the primary point of coordination for the family, ensuring communication and accountability among team members. Advocate with youth and families within complex systems to reduce barriers to care. Develop and maintain proactive safety plans in collaboration with youth, families, and team members and ensure the team has copies to support the youth. Update the plan at every change in client's clinical status. Provide services in the community, home, school, and natural environment with flexibility in scheduling to meet the needs of the youth and families including evenings and non-traditional hours. Track progress toward goals and adjust plans as necessary. Collect and report required program and outcome data. Participate in fidelity monitoring and quality improvement activities including coaching and supervision. Maintain timely, accurate, and complete documentation in the EHR using required tools in compliance with policy and regulatory standards. Ensure all services align with high-fidelity wrap around principles and program expectations. Communicate changes in clinical presentation or needs to the treatment team.EDUCATION
Have a minimum of a bachelor's degree from an accredited four (4) year institution of higher learning with a specialization in psychology, mental health and human services, social work, special education, counseling, sociology, nursing, rehabilitation, sociology, or a closely related field OR have a bachelor's degree from an accredited four (4) year educational institution in an unrelated field and at least one (1) year of full-time equivalent relevant human services experience Completion of the approved Wraparound training within fifteen (15) months from date of hire.CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED
Valid state driver's license.EXPERIENCE
Experience working in or with a primary care clinic with medical staff strongly encouraged.KNOWLEDGE AND SKILLS
Knowledge in some or all of the following areas: Child development, family systems, special education, mental health, developmental delays, juvenile justice system.
