Job Summary We are seeking a licensed Physical Therapist (PT) to provide in-home evaluations and treatments for patients across Pasco and Pinellas counties. This role offers autonomy, flexibility, and the opportunity to make a meaningful impact in patients' daily lives. Responsibilities Perform in-home PT evaluations and develop individualized care plans. Provide therapeutic interventions, gait training, strengthening, and mobility work. Document visits accurately and efficiently in WellSky EMR. Communicate with physicians, nurses, and agency staff. Educate patients and caregivers on exercises and safety. Adjust treatment plans based on patient progress. Travel to patient homes within the service area. Qualifications Active Florida Physical Therapist (PT) License. Home health experience preferred (not required). Strong assessment and documentation skills. Familiarity with WellSky EMR is a plus. Reliable transportation. Strong communication and professionalism.
