$20•23/hour, with opportunity for growth About Premier Host Homes Premier Host Homes is an Iowa HCBS Waiver provider supporting individuals with intellectual and developmental disabilities through our Host Home model. We are a small, close-knit administrative team that genuinely cares about the people we serve and the people who provide that care . We are growing in the Des Moines area and are looking for an IC Manager who shares those values. This is not a position for someone who needs to be micromanaged. We are looking for someone who can think critically, work independently, build strong relationships, solve problems, stay extremely organized, and become a trusted resource for our Host Home Providers and members. The Role The IC Manager provides direct oversight and support to Host Home Providers (HHPs) and members receiving HCBS services . For many of the people you work with, you will be the face of Premier . You will be the first point of contact when a Host Home Provider has a question, needs guidance, encounters a challenging situation, or simply needs help figuring out what comes next. This position requires someone who can balance people, compliance, documentation, problem-solving, advocacy, and relationships . Our ideal IC Manager doesn't just identify a problem—they help find the solution. Primary ResponsibilitiesHost Home Provider & Member Support Serve as the primary point of contact and resource for assigned Host Home Providers. Develop positive, professional relationships with members, providers, guardians, case managers, and other members of the interdisciplinary team. Advocate for both the member and Host Home Provider while maintaining appropriate professional boundaries and ensuring services remain member-centered. Address concerns promptly and help providers navigate challenging situations. Assist providers with identifying resources and supports when needed. Provide guidance related to challenging or problem behaviors and collaborate with the team to identify appropriate strategies. Participate in member meetings, IDT meetings, staffing meetings, and other meetings as needed. Assist with developing meaningful, individualized member goals. Help ensure members receive high-quality services while maintaining dignity, choice, independence, and rights. Documentation & Compliance Review service documentation for quality, accuracy, completeness, and compliance. Regularly review Therap and Rippling for member and provider documentation and compliance requirements. Ensure assigned Host Home Providers remain current with required trainings, certifications, documentation, and other state or agency requirements. Monitor member records and ensure required plans and documentation remain current. Update member plans, goals, rights restrictions, and related information as needed. Set up new Host Home Providers and members in Therap. Review and approve Host Home Provider timesheets. Complete assigned Quality Improvement (QI) activities. Identify documentation or compliance concerns and follow through until they are resolved. Maintain working knowledge of Iowa HCBS requirements and stay current as regulations, policies, and expectations change. Home Visits & Oversight Complete required monthly visits with assigned members and Host Home Providers. Travel throughout the assigned service area, which may include locations up to approximately two hours away . Assess the overall home environment, member well-being, provider needs, documentation, compliance, and any concerns requiring follow-up. Communicate concerns promptly and ensure identified issues are addressed. A company vehicle is provided for required work travel. Preferred Experience Experience in one or more of the following areas is strongly preferred: Intellectual and developmental disabilities HCBS Waiver services Mental or behavioral health Supporting individuals with challenging behaviors Case management or care coordination Residential services Direct support services Human services Management or supervisory roles Documentation and regulatory compliance Previous experience with Therap is a plus, but we are willing to train the right person. Schedule & Work Environment This is primarily a work-from-home position with required travel for member/provider visits, meetings, events, and other business needs. The typical schedule is Monday through Friday with flexibility in how you structure your day . We believe professionals should be able to manage their own schedules. Get your work done, meet your responsibilities, communicate with your team, and hit your hours. Because this is a professional work-from-home position requiring meetings, phone calls, documentation, and focused work, employees must have reliable childcare during working hours when applicable.
