Help adults rebuild stability, strengthen independence, and move toward their goals in a supportive short-term recovery setting. Make a Meaningful Difference Community Support Network (CSN) is seeking a compassionate, dependable Recovery House Care Manager to support adults living with behavioral health challenges. In this hands-on role, you will combine care management, skill building, advocacy, crisis support, and positive household engagement to help program participants move toward greater wellness and independence. You will join the Housing and Wellness Program and work in a client-centered environment guided by safety, dignity, integrated compassion and accountability, and strengths-based growth. This position reports to the Program Manager.

What You Will Do:

Build trusting, respectful relationships with program participants and provide nonjudgmental, strengths-based guidance. Conduct outreach, interviews, intake, discharge, and needs assessments for prospective and current participants. Partner with up to eight program participants to create and carry out individualized care plans. Provide ongoing care management, including help with benefits, identification documents, employment resources, housing applications, health care, substance-use services, and other community supports. Use evidence-based practices, including Motivational Interviewing, to support growth, independence, and social responsibility. Provide crisis prevention, intervention, and management when needed. Lead educational groups, encourage recreational activities, and support positive, cooperative participation in the household. Schedule appointments, monitor referrals and follow-up needs, respond to messages, and coordinate with community providers. Transport residents in agency vehicles as needed and help participants access community resources. Maintain timely, accurate documentation of assessments, services, referrals, advocacy, counseling, crisis response, and special incidents. Help maintain a welcoming, safe, comfortable, and non-institutional home environment. Participate in supervision, staff meetings, training, food bank shopping, and program planning.

What You Bring:

High school diploma or equivalent. At least one year of experience working with adults who have mental health or behavioral health challenges, or equivalent relevant experience. A calm, respectful, and dependable approach, including strong observation and listening skills. The ability to stay focused, organized, and responsive in an environment where priorities may change quickly. Working knowledge of Microsoft Word, Excel, and standard computer programs. Reliable transportation, a valid California driver's license, and the ability to qualify under CSN's automobile insurance requirements. The ability to satisfy all programrequirements, including the required criminal background clearance. Preferred Experience College coursework in social services or comparable experience in the social services field. Knowledge of social rehabilitation principles and current best practices. Knowledge of Sonoma County Behavioral Health services and local community resources. Experience collaborating within a team-based approach to treatment and program planning.

Physical Requirements:

This role includes occasionally lifting or carrying supplies up to 20 pounds; occasionally using stairs or a ladder; reaching at high and low levels; seeing objects at a distance; and walking with participants for up to two miles at a time.
