Full Job Description Full-Time and Part-Time Shift Lead:
Pay up to $24 per hour 1. Assists Director of Crisis Services with clinical and administrative oversight of programs and services; including staffing shifts, filling job vacancies, supervising shift leads. 2. Monitors quality, completion and timely submission of clinical assessments, treatment plans, outcome measurements, discharge summaries and other required documentation and reports. 3. Assists Director of Crisis Services in providing on-going support, instruction, and supervision to Shift Leaders on all shifts. 5. Supports shift leads in the completes and documents the treatment/service plan at the time the client enters the service. Work with team members for treatment planning including describing strengths, preferences and needs; and identifying treatment goals and strategies for each client assigned. 6. Ensures that all new staff are properly oriented upon hire and that they participate in agency training as required. Re-orients existing staff on paperwork and unit requirements as needed. 6. Provides training for staff on documentation requirements and ensures that all staff meet no less than the minimum requirements for documentation standards. Assists Director of Crisis Services with ensuring that Supervision Plans and Logs are completed per requirements. 7. Assists Director of Crisis Services with disseminating pertinent information to all staff. 8. Provides training to team members with a focus on consistency across shifts. 9. Assist shift leads with case support by identifying additional services needed for client care, coordinating, and collaborating with relevant services, preparing, and discussing discharge plans with each client assigned. 10. Works with team to ensure shift notes, 15-minute bed checks, and all other required shift duties are complete and documented before end of shift. 11. Ensuring that necessary documentation within Credible is done so that UM staff can do what needs to be for the MCO's. 12. Communicating with UM Staff of all new admits and discharges and ensuring that discharge summaries are completed within Credible. 13. Conducts chart reviews monthly and provide feedback to shift leads as needed.
