Jobs › Danvers, MA › Human Services Coordinator II State of Massachusetts Human Services Coordinator II Danvers, MA Apply The Department of Developmental Services (DDS) is excited to offer a career growth opportunity to a motivated and talented individual for the Human Services Coordinator Supervisor (HSC II) position in the Northeast Area Office for the Acquired Brain Injury program known as ABI/MFP (Acquired Brain Injury/Moving Forward Plan) located in Danvers, MA. The Human Service Coordinator Supervisor will work with assigned staff and the Area Office Supervisory Team to build respectful and collaborative partnerships with individuals, families, guardians, provider agencies and other stakeholder groups involving the work of the Area Office and Department. The Human Service Coordinator II provides direct supervision, support, consultation, and orientation to assigned Human Service Coordinators. The incumbent will help coordinate and facilities he development of Individual Support Plans (ISPs). The HSC (II) will function as the Service Coordinator Supervisor for the ABI/MFP Program. Additional Human Service Coordinator II responsibilities include:
- Responsible for providing direct supervision to at least five Service Coordinators on the Regional ABI/MFP team. Support and monitor the Service Coordinators' performance and ensure compliance with statewide guidelines.
- Serves as a core member of the ABI/MFP leadership team. Participates in planning, policy development and resource allocation activity.
- Collaborates with ABI/MFP leadership team on newly eligible participants, residential referrals, and complex clinical and medical cases
- Partners with regional Skilled Nursing Facilities and ABI/MFP team members to transition individuals served by the waiver to the community
- Assigns and monitors cases to Human Service Coordinators; serves as liaison for individuals entering the ABI/MFP waiver system and compiles, maintains, and prepares individual's statistics and resource service data
- Facilitates collaboration and discharge planning with UMASS Medical School, Providers Network, MassAbility, Skilled Nursing Facilities, MFP Demonstrations Projects Office, and Transition Entities
- Supports Service Coordinators to involve regional resources; risk management, human rights, neuropsychological, psychological, medical, OT/PT/SLP, DDS legal etc.
- Oversee the service coordinators monitoring of the implementation of individual services provided and maintains regular and consistent contact with individuals/guardians, service providers and families as indicated.
- Oversee the maintenance of individual records and ensure complete and accurate information including compilation and preparation of case statistics and resource/service data
- Oversee special projects as assigned and coordinate and facilitates the development of an Individual Support Plan that incorporates a broad spectrum of individual needs, skills, and desires.
- Participates in a training role, including field interagency and provider training _

Substitutions:
I. A Bachelor's degree or higher in a related field may substitute for two (2) years of the required (A) experience. Based on assignment as Qualified Intellectual Disabled Professionals, within the Department of Developmental Services, a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, physical education or other a related field is required. Salary placement is determined by a combination of factors, including the candidate's years of directly related experience and education, and alignment with our internal compensation structure as set forth by the Human Resources Division's Hiring Guidelines.
