In this vital role, you will provide comprehensive assistance with daily activities, health monitoring, and emotional support within clients' homes or community settings. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient health by checking vital signs, observing symptoms, and reporting changes promptly to healthcare professionals Administer medications accurately following prescribed care plans while maintaining compliance with HIPAA regulations Prepare nutritious meals aligned with dietary requirements and assist with feeding as needed Operate medical equipment such as Hoyer lifts and assistive devices to ensure safe patient transfers and mobility Maintain a clean and safe environment by performing light housekeeping tasks including cleaning, laundry, and organizing personal spaces Support clients with cognitive challenges such as dementia or Alzheimer's through memory care techniques and patient engagement strategies Document all care activities meticulously in accordance with agency protocols and state regulations Collaborate with healthcare teams to develop and implement personalized care plans tailored to each individual's needs Provide emotional support and companionship to promote mental well-being and social engagement Qualifications Proven experience in home health, assisted living, nursing home, or group home settings; long-term care experience preferred Certification in First Aid and CPR; additional training in hospice or dementia care is a plus Knowledge of HIPAA regulations to ensure patient confidentiality at all times Ability to work with individuals with developmental disabilities, autism, or other special needs populations Strong observational skills for patient monitoring and safety assessments Physical ability for heavy lifting, assisting with transfers using Hoyer lifts, and supporting mobility needs Excellent communication skills to effectively collaborate with clients, families, and healthcare providers Compassionate attitude combined with a proactive approach to caregiving responsibilities Embark on a rewarding career where your energy fuels positive change!
