Job Description

At MPA, we are dedicated to providing compassionate, comprehensive care for children and their families. As a Patient-Centered Medical Home (PCMH)-certified practice, we emphasize a team-based approach to care, ensuring every patient receives personalized and coordinated healthcare. We are seeking a skilled and dedicated Pediatric Licensed Practical Nurse (LPN) to join our team. The ideal candidate will need to have significant experience in triage and PCMH workflows, bringing their expertise to support our mission of quality pediatric care.
Key Responsibilities:
Triage:
Conduct telephone triage, assessing patient needs and providing guidance or escalation to providers as appropriate. Perform in-office patient assessments, documenting medical histories, symptoms, and concerns. Monitor and prioritize patient care needs to ensure timely attention.
PCMH Duties:
Implement and maintain PCMH protocols, ensuring compliance with standards and best practices. Act as a liaison to support care coordination, including referrals, follow-ups, and communication with external healthcare providers. Assist in managing care plans for high-risk or chronic-condition patients. Participate in quality improvement initiatives to enhance PCMH delivery.
General Duties:
Administer medications, immunizations, and treatments as prescribed by providers. Educate patients and families on health conditions, treatment plans, and preventive care. Maintain accurate and up-to-date patient records in the electronic medical record (EMR) system.
Qualifications:
Current and valid LPN license in CT. Demonstrated experience with PCMH workflows and requirements (certification or significant hands-on experience). Strong triage skills, with the ability to assess and prioritize care efficiently. Familiarity with EMR systems and healthcare technology. Excellent communication and interpersonal skills, with a compassionate approach to patient care.

$28.00 - $32.00 per hour Expected hours: 40 per week
