Job Description
Overview Join our dynamic healthcare team as a Licensed Practical Nurse (LPN) where your skills and compassion will make a real difference in the lives of our patients! In this vital role, you will provide essential nursing care under the supervision of registered nurses and physicians, ensuring that every patient receives the highest quality of care. Your expertise will shine as you work in long-term care facilities while fostering a supportive environment for patients and their families. What you'll do Deliver compassionate patient care by performing physical examinations and assessments. Administer medications and treatments as prescribed, ensuring adherence to safety protocols. Collaborate with healthcare teams to develop and implement individualized patient care plans. Monitor vital signs and report changes in patient conditions to the supervising nurse or physician. Assist with procedures such as IV infusion, catheterization, and wound care. Provide education to patients and families regarding health management and wellness strategies. Maintain accurate medical documentation in electronic health record (EHR) systems while adhering to HIPAA regulations. Participate in discharge planning and case management to ensure continuity of care. Basic qualifications Valid LPN license in the state of practice. Strong knowledge of medical terminology, anatomy, and physiology. Proficient in basic life support (BLS) techniques. Preferred qualifications Experience with electronic medical record (EMR) systems. Background in critical care, geriatrics, or pediatrics is highly desirable. Familiarity with ICD-10 coding and medical documentation practices. Previous experience working with individuals with developmental disabilities or in memory care settings.
