Medical Social Worker - Home Health

Job Summary The Medical Social Worker provides psychosocial support, care coordination, and resource navigation for patients and families receiving medical services in the Zephyrhills area. This role works closely with physicians, nurses, case managers, and community agencies to ensure patients receive comprehensive, person‑centered care that addresses medical, emotional, social, and environmental needs. Key Responsibilities Conduct comprehensive psychosocial assessments to identify patient needs related to health, safety, mental well‑being, and social determinants of health. Develop individualized care plans in collaboration with the interdisciplinary medical team. Provide counseling, crisis intervention, and emotional support to patients and families coping with illness, hospitalization, or life‑changing diagnoses. Coordinate discharge planning, including placement in rehabilitation, skilled nursing, home health, or community‑based programs. Connect patients with community resources such as financial assistance, transportation, housing support, and behavioral health services. Advocate for patient rights and ensure culturally competent, trauma‑informed care. Facilitate family meetings, care conferences, and communication between patients, caregivers, and providers. Document assessments, interventions, and care plans in accordance with regulatory and organizational standards. Participate in quality improvement initiatives and interdisciplinary team meetings. Maintain knowledge of local resources in Pasco County and the broader Tampa Bay region. Qualifications Master's degree in Social Work (MSW) from an accredited program required. Current Florida licensure (LCSW preferred; registered clinical social work intern accepted depending on employer). Experience in a healthcare setting such as hospital, hospice, home health, or outpatient clinic strongly preferred. Knowledge of medical terminology, chronic illness management, and psychosocial impacts of disease. Strong communication, problem‑solving, and crisis‑management skills. Ability to work collaboratively in a fast‑paced clinical environment. Preferred Skills Experience with Medicare/Medicaid and Florida community resource systems. Familiarity with electronic health records (EHR). Bilingual English/Spanish or English/Creole is a plus in the Zephyrhills region.

$30.00 - $40.00 per hour
