Job Description

Position Summary The Nurse Practitioner (NP) provides high-quality, patient-centered care in a clinical setting. This role is responsible for the evaluation, diagnosis, treatment, and follow-up care of patients within the NP scope of practice. The NP supports a collaborative, team-based care model focused on improving patient outcomes and experience. Duties & Responsibilities Clinical Care Perform comprehensive patient assessments, including medical history and physical examinations Develop and implement evidence-based treatment plans Order, interpret, and follow up on laboratory tests, imaging, and diagnostic studies Prescribe medications (including controlled substances) in accordance with California regulations Manage chronic conditions such as diabetes, hypertension, COPD, and CHF Evaluate and treat acute conditions (e.g., colds, flu, infections, minor injuries, rashes) Initiate referrals and coordinate care with specialists Deliver culturally competent care to diverse patient populations Collaborate with clinical staff to optimize patient outcomes Care Coordination & Patient Management Ensure continuity of care through follow-up visits, chart review, and care coordination Develop and manage treatment plans, referrals, and prescriptions within scope of practice Collaborate with supervising or partnering physicians per standardized procedures Participate in interdisciplinary care planning Support preventative care programs such as Annual Wellness Visits (AWVs) and chronic disease management Maintain accurate, timely, and compliant documentation in the EMR Compliance & Quality Adhere to established clinical protocols and standardized procedures Maintain compliance with HIPAA, OSHA, infection control, and regulatory requirements Participate in quality improvement initiatives, chart audits, and peer reviews Meet performance and quality metrics (e.g., HEDIS measures, immunizations, RAF accuracy, AWVs) Patient Experience Provide compassionate, respectful, and culturally sensitive care Educate patients on treatment plans, medications, and preventative health Promote a positive, team-oriented environment Manage time effectively to reduce patient wait times Support colleagues during high patient volume to ensure timely care delivery Leadership & Collaboration Serve as a clinical resource for medical assistants, LVNs, and support staff Participate in staff meetings, training, and workflow improvement initiatives Collaborate with clinical and operational leadership across sites as needed Perform additional duties as assigned Required Qualifications Active California Nurse Practitioner (NP) license National board certification (e.g., FNP, AGNP) DEA license and furnishing number with Schedule II authority (or eligibility) Current BLS/CPR certification Minimum 1-2 years of primary care experience preferred Preferred Qualifications Bilingual (English/Spanish) Experience with family practice, geriatrics, or senior-focused care Familiarity with HMO/IPA models and risk adjustment coding (RAF, HCC) Experience in multi-site or high-volume clinical environments Physical & Work Environment Requirements Ability to stand, walk, bend, and sit for extended periods throughout the day Ability to lift up to 20-30 lbs as needed Work performed in a clinical environment, including exam rooms and medical offices Exposure to moderate noise levels and infectious materials Ability to safely assist patients with mobility and transfers Additional Requirements To perform this role successfully, an individual must be able to carry out all essential duties effectively.
