ATD. ROLE SUMMARY

_______________________________ ATD is looking for a hard-working, scrappy, and mission-aligned Outreach Associate to ensure we meet the needs of our growing market. This role is all about building relationships, earning trust, and helping more New Yorkers—especially those in the Chinese community —access critical in-home care services. The Outreach Associate will split their time between reactivating former ATD patients and generating new referrals through in-house events and follow-up outreach . You will be a visible, trusted representative of ATD—rebuilding relationships, hosting and attending events, and ensuring patients understand how ATD can support their care.

WHAT YOU'LL NEED TO DO

______________________________ Patient Reactivation & Engagement (50%)

• Own patient reactivation goals by outreach to former or inactive ATD patients via phone calls, in-person visits, and community touchpoints
• Rebuild trust and relationships with former patients and families to understand current needs and barriers to re-enrollment
• Plan and host small group events, informational sessions, and community gatherings designed to re-engage former patients
• Partner closely with Intake teams to ensure smooth re-enrollment and service restart
• Track reactivation outreach, outcomes, and trends to improve engagement strategies New Referral Generation & Follow-Up (50%)
• Represent ATD at large community events, health fairs, and cultural gatherings across NYC (e.g., Flushing, Sunset Park, Elmhurst, Chinatown)
• Proactively engage attendees, collect referral leads, and educate families about ATD's in-home care services
• Follow up consistently with event-generated referrals to convert interest into active admissions
• Share field insights and referral feedback with Strategy and Intake teams
• Track outreach activities and referrals in internal systems
• Assist with maintaining outreach records and event documentation

QUALIFICATIONS & EXPERIENCE

• 1+ year experience in outreach, field sales, community organizing, or relationship management (healthcare or homecare background strongly preferred)
• Excellent customer service over the phone and in-person
• Proven ability to build genuine relationships and influence diverse stakeholders
• Comfortable with autonomy and accountability in a fast-paced field environment
• Fluent in English and Mandarin or Cantonese required
• Proficient in CRM tools (e.g., Salesforce), Google Workspace, and Excel
• Must be able to travel either with or without reasonable accommodations

Range:

$22-26 / hour

LOCATION

_________________________________________________ Flushing, New York (~30-45 minutes from Manhattan). This person will be in the office 5 days a week or out hosting events.

Pay:

$22.00 - $26.00 per hour
