Key Responsibilities:

Perform annual and post-discharge home health risk assessments for Medicare Advantage members Review and assess patients' medical history, current health conditions, and medication use Educate patients on their medical conditions and prescribed medications Identify gaps in care and provide recommendations to address them No requirement to develop treatment plans or prescribe medications

Qualifications:
