Physical Therapist for Medicare Home Health Visits

Job Overview We are seeking a dedicated and energetic Physical Therapist (PT) for Evaluations and for supervisoty visits to join our team, providing vital Medicare home health visits to patients in their own home. As a key member of our healthcare team, you will assess patient needs, develop tailored care plans, and execute therapy interventions across diverse settings including home health, outpatient clinics, and inpatient facilities. Your expertise will help bridge the gap between hospital care and community living, ensuring seamless transitions and optimal patient outcomes. Responsibilities Conduct comprehensive assessments of patients' physical conditions, including mobility, strength, and functional abilities, utilizing physiology and anatomy knowledge. Develop and implement individualized care plans that incorporate physiotherapy techniques such as trigger point therapy, orthopedics, geriatrics, pediatrics, and outpatient therapies. Perform therapeutic interventions aimed at improving patient mobility, reducing pain, and restoring function through evidence-based practices. Collaborate with interdisciplinary teams to coordinate discharge planning and ensure continuity of care across inpatient, hospital, home health, and outpatient settings. Educate patients and caregivers on exercises, safety measures, and health management strategies to promote independence at home and in the community. Document all patient interactions accurately within electronic medical record (EMR) systems while adhering to regulatory standards. Assist in managing complex cases involving acute care scenarios or post-surgical rehabilitation by applying specialized knowledge of medical terminology and care protocols. Experience Proven experience in inpatient hospital environments or outpatient clinics with a focus on geriatrics Demonstrated ability to perform discharge planning effectively while coordinating with healthcare teams for smooth patient transitions. Strong background in home & community care experience with familiarity in home health services delivery. Knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques essential for effective patient treatment. Experience utilizing EMR systems for documentation and care coordination. Prior work in acute care or hospital settings is highly desirable to handle complex cases efficiently. Certifications or training in trigger point therapy or specialized physiotherapy modalities are a plus.
