Job Description

Referral Specialist (Full Time )
Location:
Mercer County General Description The Referral Management Department is designed to minimize the inefficiencies in healthcare delivery that can occur within the system. Referral Management Specialists are accountable for processing and following patient referrals and testing to completion by working collaboratively, encouraging patient compliance, performing outreach, ensuring timely feedback and confirming completion of orders to minimize risk and help eliminate gaps in care with the use of computerized databases and electronic records. Essential Functions
  • Review and process provider orders by gathering necessary records
  • Obtain insurance authorizations
  • Collaborate with providers, patients, and servicing facilities
  • Build and maintain relationships with external facilities for communication and record exchange
  • Document information in EMR and Referral Management System
  • Perform outreach via phone, fax, and email
  • Verify appointment attendance and obtain correspondence from external facilities
  • Assist with resolving barriers such as financial aid, transportation, and language interpretation
  • Work with clinical staff on same-day emergent orders Qualifications
  • High school diploma or equivalent required
  • Certification, associate degree, or healthcare related coursework required
  • 2+ years' experience in a healthcare setting required
  • Strong customer service focus
  • Effective verbal and written communication skills
  • Teamwork orientation
  • Organized and able to manage competing priorities
  • Able to take and follow through with delegated tasks and accountability
  • Ability to critically think and problem solve
Location and Hours of Position PHN Referral Management Department:
62-66 E State Street Sharon Pa 16146 The general office hours for this position are Monday - Friday 8:30am - 5:00pm. Compensation Position starts at $16.00 per hour, however, PHN recognizes certification, licensure, experience, education and site location when determining wage.
