What You'll Be Doing:

Complete detailed Start of Care assessments for new patients Submit accurate, timely OASIS documentation Build individualized care plans aligned with patient goals and home environments Educate patients and caregivers on expectations, medications, and safety Collaborate with therapists, case managers, and referring providers Serve as the clinical leader who sets the tone for exceptional home health services

What You Need:
