Registered Nurse (RN) - Home Health

Job Description Help for Job Description. Opens a new window. Matrix Home Care is seeking compassionate and skilled Registered Nurses (RNs) to join our growing home health team. This role provides direct care in the home setting, including Start of Care (SOC), routine visits, and care management. Nurses must be flexible and able to support multiple service lines based on patient needs.

Key Responsibilities:

Perform Start of Care (SOC) visits including OASIS assessments. Provide skilled nursing visits and ongoing patient care in the home. Develop and manage individualized care plans. Monitor patient conditions and adjust care as needed. Educate patients and families on disease management and care plans. Complete accurate and timely documentation in EMR system (KanTime preferred). Coordinate care with physicians, therapists (PT, OT, Speech), and interdisciplinary team. Ensure compliance with Medicare and home health regulations. Under Florida law, this role is subject to background screening per Chapter 435, Florida Statutes, including fingerprinting and review for eligibility before employment.
