Job Description
- Build and maintain professional relationships with physicians, hospitals, skilled nursing facilities, and community partners.
- Educate referral sources on home health and hospice services, eligibility criteria, and benefits of care.
- Identify opportunities to grow referrals and expand community presence.
- Conduct in-person visits, presentations, and outreach activities within the service area.
- Collaborate with clinical teams to ensure appropriate patient referrals and smooth transitions of care.
- Track and report outreach activities, referral trends, and market feedback.
- Represent the organization professionally in the community and at networking events.
- Maintain knowledge of services, programs, and regulatory guidelines. Qualifications
- Healthcare background required (examples include CNA, LPN, RN, Medical Assistant, COTA, or other clinical/health-related experience).
- Previous experience in healthcare marketing, outreach, and business development.
- Strong understanding of home health and hospice services.
- Excellent interpersonal, communication, and presentation skills
- Highly self-motivated, proactive, and results-driven with a "go-getter" mindset.
- Demonstrated ability to independently identify opportunities, initiate outreach in new settings, and build relationships from the ground-up.
- Strong follow-through skills with the ability to maintain and grow relationships over time.
- Ability to build and maintain professional relationships.
- Strong organizational skills with attention to detail.
- Valid driver's license and proof of current auto insurance. Schedule
- Full-time, Monday-Friday (8:30 AM- 5:00 PM)
