Job Description
Compassion Health Care is seeking a dynamic individual for a Clinical Care Coordinator whose primary role is to work directly with one of our Primary Care Providers at our new Madison location. The Clinical Care Coordinator needs to possess the characteristics of great work ethic, sound clinical judgment, team player, and demonstrates our core values of compassion for others, commitment, integrity and excellence in their daily work.
Operational Support & Management Coverage:
Provides management-level operational support and oversight to ensure the safe and efficient operation of the facility. Responds to facility or environmental emergencies, operational outages or disruptions, staff emergencies, patient volume or scheduling concerns requiring additional resources, and situations requiring management assistance or de-escalation, including concerns involving irate or escalated patients. Coordinates vendor access and activities in collaboration with site management and provides additional support as needed. Conducts facility rounds, monitors patient flow and overall operational readiness, and remains available to staff for questions, concerns, and immediate operational needs. As a Clinical Care Coordinator at Compassion Health Care, you will work as part of a healthcare delivery team serving patients of all ages. The Clinical Care Coordinator utilizes the nursing process including assessment, planning, implementation evaluation and recording of patient care and patient education. The position provides support to a Primary Care Provider by anticipating needs and providing assistance in the provision of care. In addition, the Clinical Care Coordinator, coordinates services with other members of the health-care team to provide patient-centered care. The role practices within the scope of the North Carolina Nurse Practice Act and/or Compassion Health Care's policies, procedures, and standards. Clinical Care Coordinators may also perform other duties across the organization as their skills and training permit and may be cross trained in other areas to accomplish operational objectives.
Job Responsibilities:
Utilizes nursing knowledge, clinical judgement, and critical thinking to provide appropriate assessments and thorough intake of patients. Assesses individual patient conditions utilizing clinical observation, medical record, verbal information, and interaction with patient/family. Identifies the impact of cultural diversity and lifestyle choices on patient condition and makes accommodations for such differences. Implements provider's orders effectively and in a timely manner. Safely administers medication as ordered by provider and documents in EMR. Consistently follows all safety policies and procedures. Ensures learning needs of patient/family are addressed and documented. Performs/assists in performing medical procedures, treatments, and tests using skilled technique and following all prescribed safety regulations to ensure accurate results. Addresses needs of patients accurately, thoroughly and within acceptable timeframe. Performs referral tracking; follows up appropriately on completed and incomplete referrals. Consistently works in a positive and cooperative manner with other employees in and outside of departmental unit.
Job Qualifications:
Diploma or associate degree from a CMA training program is required; clinical experience in primary care setting is preferred. Current/valid CMA certification required. Basic Cardiac Life Support (BCLS) is required. Basic computer skills required; prefer experience with Electronic Medical Record. The selected candidate must meet CHC's employment eligibility requirements, including but not limited to, successfully completing pre-employment drug testing and criminal background screening.
•Please note before applying that this is a M-F, no holidays or weekends position, in a family practice setting, and the pay is based on those factors, as well as the relevant experience of the candidate. Medical Assistant starts at $21.35 with no experience Excellent Benefit package includes employee health insurance, dental insurance, LTD, and life insurance premiums covered by the company, 11 paid holidays, generous PTO, and work week ends at noon on Fridays.
