Position Overview The Community Liaison serves as the primary ambassador for Targeted Case Management and Behavioral Health Services across the IL markets. This role drives growth by building referral relationships, executing targeted outreach, and representing the organization at community events. The liaison will focus on generating leads and enrollments for Targeted Case Management, Therapy, and Medication Management Services. Key Responsibilities Outreach & Relationship Development Build and maintain partnerships with healthcare providers, hospitals, social service agencies, senior communities, faith-based organizations, and key referral sources across the territory. Represent the organization at local networking events, chamber meetings, health fairs, senior expos, and community workshops. Conduct face-to-face and virtual meetings with decision-makers to educate them on service offerings, eligibility criteria, and enrollment processes. Marketing & Business Development Execute targeted outreach strategies to increase brand awareness and drive client inquiries for case management, therapy, and medication management. Collaborate with internal team to develop resonant campaigns, materials, and presentations. Monitor healthcare trends, competitor activities, and community needs to refine outreach strategy. Lead Generation & Enrollment Support Identify and activate new referral sources across medical and non-medical verticals. Guide prospective clients and families through eligibility screening, benefit enrollment, and voluntary alignment processes (as applicable). Support smooth transitions from inquiry to service initiation, coordinating with operations and care management teams. Tracking, Reporting & Compliance Maintain accurate records of outreach activities, referral source engagement, and lead conversions using CRM tools. Meet or exceed monthly performance objectives for meetings, referrals, and enrollments. Adhere to all company policies, HIPAA guidelines, and applicable marketing/ outreach regulations. Non-Essential Duties Performs other duties as assigned. The ideal candidate must be able to work independently, remain self-directed and flexible, manage a high-volume workload with accuracy, and function with minimal supervision. Qualifications Education & Experience Bachelor's degree in Marketing, Healthcare Administration, Communications, or related field preferred. 2+ years of experience in business development, healthcare outreach, community relations, or B2B sales (home care, senior services, behavioral health, or Medicaid-focused programs strongly preferred). Demonstrated success building referral relationships and meeting performance targets. Skills & Competencies Excellent interpersonal, communication, and presentation skills; comfortable engaging diverse stakeholders. Strong organizational and time-management abilities; self-motivated with minimal oversight. Proficiency with Microsoft Office (Word, Excel, Outlook) and CRM platforms. Valid driver's license and reliable transportation required for field travel. Preferred Knowledge Familiarity with Medicaid, Medicare, and commercial insurance eligibility/enrollment processes. Understanding of enhanced care management, case management, therapy, and medication management service models. Work Schedule & Travel Full-time, Monday-Friday standard hours. After-hours and weekend availability required to cover community events, health fairs, and stakeholder meetings. Regular travel across the IL Market; mileage reimbursement provided.
