About us Northstar CFSS is a Minnesota DHS-contracted CFSS consultation agency. We help people across Minnesota choose and use Community First Services and Supports (CFSS): model education, Service Delivery Plans, and ongoing consultation support. About the role You'll work directly with CFSS participants and their families. You'll explain their CFSS options, help write and review Service Delivery Plans, document each session, and support people under our Lead Employee. Sessions are in person and/or remote across Minnesota. New consultants complete orientation and shadowing before taking their own caseload. This is a consultation staff role, not a participant support-worker (PCA) job. Languages (preferred, not required) We serve families in their own language. Bilingual candidates who speak Spanish, Arabic, Lao, Karen, Vietnamese, or another language spoken in Minnesota communities are strongly preferred. (We're currently covered for Hmong, Somali, Oromo, and Amharic.) English is required for documentation and DHS systems. Responsibilities

• Educate participants and representatives on CFSS so they can make informed choices
• Help write and review Service Delivery Plans and upload them to MnCHOICES
• Document consultation sessions (DHS-6893S or equivalent)
• Communicate with case managers and agencies
• Complete all DHS-required training and keep credentials current
• Follow conflict-of-interest rules under Minn. Stat. §256B.85 Required qualifications
• Age 18 or older
• Pass a Minnesota Chapter 245C background study (NetStudy 2.0) before providing services
• Meet DHS education requirements for consultation staff, OR an allowed substitution:
• One year of full-time direct service to people with disabilities or people over 65; OR
• Experience coordinating or directing services for people with disabilities or people over 65, including self-directed services; OR
• Experience coordinating your own services
• Strong English for documentation Preferred
• Bilingual (see languages above)
• Lives in Minnesota
• Experience with CDCS, support brokering, case management/care coordination, or FMS/PCA agency work
• Valid driver's license and reliable transportation for field visits Pay Paid per completed consultation session.
