Psychosocial rehabilitation Services is a structured treatment service in a licensed facility for adults that builds on strengths and addresses identified needs. This medically necessary service directly addresses the individual's diagnostic and clinical needs, which are evidenced by the presence of a diagnosable mental, behavioral, or emotional disturbance (as defined by the DSM-5 or any subsequent editions of this reference material), with symptoms and effects documented in a comprehensive clinical assessment and the PCP. The Psychosocial Rehabilitation Service facilitator can be an Associate Professional or Qualified Professional who is responsible for implementing, coordination and assisting in providing therapeutic and rehabilitative services to consumers enrolled in Psychosocial Rehabilitation (PSR).
Position Requirements:
Bachelor Degree in human services with at least 1 year post bachelor experience working with adults with severe and persistent mental illness OR Masters Degree in human services with less than 1 year post graduate experience working with adults with severe and persistent mental illness AND No substantiated findings of abuse or neglect listed on the
North Carolina Health Care Personnel Registry or Central Registry Duties and Responsibilities:
Provides psychoeducation as indicated in the PCP Provide crisis interventions Provide behavioral and substance use disorder treatment interventions Assists with the development of relapse prevention and disease management strategies Participates in the initial development, implementation, and ongoing revision of the PCP Communicates the individual's progress and the effectiveness of the strategies and interventions as outlined in the PCP Provides case management Linkage and referral to formal and informal supports Monitoring and follow up Provides direct services according the consumer's Person Centered Plan and regulatory requirements of Clinical Coverage Policy 8A. Utilize interventions to promote consumer recovery, symptom stability, increased coping skills and achievement of the highest level of functioning in the community. Develops and maintains collaborative working relationships with community partners on behalf of consumers and their recovery goals. Assist consumers in accessing and effectively using community resources. Assist consumer in securing transportation as needed to ensure that community access. Cooperatively and actively participate in all assigned staff and supervisory meetings as well as in service trainings and staff development activities. Work with supervisor to self-identify training and supervision needs, and ensure services to clients are provided only within the scope of current expertise and abilities Cooperatively performs other job duties as assigned to support the provision of quality services to consumers; remain available to team members in crisis situations Maintain client confidentiality in adherence to HIPPA regulations. Complete in a timely manner, accurate clinical documentation of all services, interventions and client related activities. Maintain this documentation in accordance with standards of AS' file management systems.
