Job Description

Job Summary The Service Coordinator - Veteran Directed Care (VDC) position plays a vital role in empowering veterans to remain independent and engaged by connecting them to supportive services, conducting assessments, and advocating for their needs. This position works collaboratively with program participants and community partners to promote stability, wellness, and quality of life. Primary Responsibilities Conduct comprehensive assessments to determine veterans' functional needs, strengths, goals, and preferences. Schedule home visits and make phone contacts per the contracted schedule. Collaborate with veterans and their representatives to develop individualized care plans that support independent living. Assist veterans in identifying and securing services such as personal care, homemaker services, respite, or assistive equipment. Provide education and guidance to veterans on their role as an employer, including hiring, training, and supervising caregivers. Work closely with the Financial Management Services (FMS) provider to ensure appropriate use of budgets and timely payment to caregivers. Monitor care plans through regular contact (in-person, phone, or virtual) to ensure services meet veterans' needs and goals. Advocate for veterans by coordinating with the VA Medical Center, community agencies, and healthcare providers. Maintain accurate, timely, and confidential documentation in compliance with VA and organizational requirements. Participate in quality assurance activities, team meetings, and training as required.
Minimum Requirements:
High school diploma or equivalent. 1 year of case management experience. Valid driver's license and reliable transportation with proof of insurance.
Preferred Requirements:
Bachelor's degree. Certified Senior Advisor (CSA), Chemical Dependency Counselor Assistant (CDCA) certification or Professional Service Coordinator (PSC) certification. Skills Experience working with older adults or vulnerable populations. Strong communication, interpersonal, and organizational skills. Knowledge of community resources and social service systems. Ability to work independently while building collaborative relationships with individuals and stakeholders. Proficiency in Microsoft Office and case documentation systems. Working Conditions Traditional office environment with standard business hours. Flexibility for telework may be approved. Conducts home visits within the 8-county region.
