Sevita is seeking a PT Caregiver to provide compassionate, person-centered support to individuals in their homes and communities. In this role, you'll assist with daily living activities such as bathing, dressing, meal preparation, and mobility, helping people of all ages and abilities live more independently. You'll follow individualized care plans, observe and report changes in condition, and engage individuals in meaningful activities. Responsibilities Provide compassionate in-home care and supervision to individuals with diverse needs Assist with daily living activities including bathing, dressing, and meal preparation Support mobility, safe transfers, and use of assistive devices as needed Monitor and report changes in physical or behavioral health to supervisors Follow individualized care plans and document care provided accurately Engage individuals in meaningful activities and community outings Administer non-clinical support in alignment with safety and company policies Collaborate with families and care teams to ensure person-centered support Maintain a clean, safe, and respectful home environment Participate in ongoing training and professional development Required Skills Personal care assistance (bathing, grooming, toileting) Safe transfer and mobility support Basic meal preparation and nutrition awareness Observation and documentation of client condition Infection control and basic safety practices Behavioral support and de-escalation basics Use of assistive devices (walkers, wheelchairs, lifts) Time management and scheduling in home-care settings Basic computer or mobile app documentation skills Knowledge of confidentiality and HIPAA basics
