Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene. Monitor patients' health by observing vital signs, recognizing changes in condition, and reporting findings accurately. Administer medications following prescribed care plans and ensure compliance with HIPAA regulations to protect client confidentiality. Support clients with meal preparation, ensuring nutritional needs are met while accommodating dietary restrictions. Provide transportation assistance for medical appointments, errands, or community outings to promote social engagement. Utilize proper manual handling techniques and equipment such as Hoyer lifts for patient transfers to ensure safety for both clients and caregivers. Maintain detailed documentation of care provided, patient observations, and any incidents in accordance with agency policies and resident's rights. Skills Proven experience working with individuals with disabilities, including developmental disabilities, neurodevelopmental disorders, or aging populations such as geriatrics. Strong knowledge of dementia care, Alzheimer's care, memory care, and hospice support strategies. Familiarity with assisted living environments, nursing homes, group homes, or home health settings. Ability to perform patient monitoring tasks such as vital signs assessment and medication administration accurately. Excellent communication skills for effectively engaging with clients, families, and healthcare teams. Competence in emergency response procedures including CPR and first aid certification.
