Job Summary/Objective:
To provide direct care to those authorized community living supports services.
Qualifications:
Minimum of High School Diploma. Good Physical Health, to include a negative reaction to a TB test. Be of good moral character, meet criminal background check criteria. Three references. Experience working with children and/or adolescents. Demonstrates ability to communicate and establish and maintain supportive relationships. Accountability The Community Living Supports Worker is directly responsible to the Family Supports Program Supervisor.
Essential Functions Travel:
Daily travel to and from appointments, activities, and errands may be required in your person vehicle. Duties To implement all policies, procedures, and or legal commitments to people we serve as required by the local CMH and Bay Human Services. Participate with all staff of Bay Human Services and local CMH in providing quality care and treatment to all people of the CLS program. Actively participate in scheduled staff meetings, workshops, and special meetings. Be an advocate for the people we serve and his/her family. Document on proper forms all activity pertaining to the care and treatment of the people in the following areas: Each contact with the person and/or family member. Each contact with local CMH personnel regarding the care and treatment of the person we serve. Timecard/sheets and other forms required for the administration of the program. Other documentation as required by local CMH, state, and federal regulations. Participate in the development of the treatment/behavior plan as appropriate. Carry out the behavior plan as directed by the Family Supports Program Supervisor and local CMH personnel. Provide and supervise activities for the person we serve as indicated in the treatment plan and/or behavior plan as directed by Family Supports Program Supervisor or local CMH personnel. Provide transportation for activities/treatment/care as appropriate and indicated in the treatment/behavior plan. Report all unusual incidents/emergency/crisis situations to the Family Supports Program Supervisor. Provide personal care if stated in the treatment/behavior plan Other appropriate duties as assigned by the Family Supports Program Supervisor.
