The Lead Direct Care Professional provides oversight, support, and direct care to individuals served by Hope Network across a variety of settings. This role serves as a leader on shift, modeling person-centered support, promoting independence, and ensuring the implementation of medical, clinical, therapeutic, and behavioral support plans. The Lead Direct Care Professional also mentors, trains, and orients new staff, assists with on-call responsibilities, and provides guidance and support to team members when supervisors are unavailable or off-site. What You'll Do Assist with daily living (ADLs), personal care, and medications Support community outings, transportation, and activities Build positive relationships and encourage independence Help maintain a safe, structured, and supportive environment On Call rotation What We're Looking For Compassionate, dependable, and team-oriented Strong communication and ability to build trust Adaptable, patient, and resilient Person-centered mindset with respect for individual differences
