Job Summary Parttime shifts available days, evenings and weekends. Approximately 14-25 hours per week. We are seeking a compassionate and dedicated Direct Care Worker to provide exceptional support and care to individuals with diverse needs in various settings, including residential facilities, assisted living communities, and home environments. In this vital role, you will promote independence, ensure safety, and enhance the quality of life for those you serve. Responsibilities Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and personal hygiene to foster independence and dignity. Monitor patients' health by observing vital signs, managing medication administration, and reporting changes or concerns promptly. Support residents with meal preparation, feeding assistance, and nutritional needs while adhering to dietary plans. Facilitate patient transfer methods Maintain accurate care plans, document patient progress, and ensure compliance with HIPAA regulations to protect client confidentiality. Provide patient personal care assistance and support with medical procedures in accordance with nursing protocols. Engage in patient observation for signs of dementia, Alzheimer's disease Assist with community outings or transportation needs safely using personal transportation assistance. Support clients in maintaining their rights by respecting preferences and promoting a respectful environment. Participate in training on CPR, first aid, and other relevant certifications to enhance care quality. Qualifications Prior experience working in group homes, nursing homes, assisted living facilities, or home health environments is highly preferred. Demonstrated ability to perform patient monitoring, observe changes in condition, and respond appropriately. Knowledge of care techniques including assisting with ADLs, medication administration, patient transfer methods, and manual handling. Familiarity with dementia care, Alzheimer's care, memory care strategies, and supporting individuals with developmental mental or intellectual disabilities. Ability to perform light cleaning duties to maintain a safe and hygienic environment. Certification in CPR and first aid is required; additional certifications such as HIPAA training. Strong communication skills for effective client interaction and team collaboration. Physical ability to lift heavy objects safely and perform manual handling tasks as needed.
