Duties Assist clients with activities of daily living (ADLs), including bathing, grooming, dressing, and mobility support. Monitor patient health by observing vital signs, medication adherence, and overall condition, reporting any concerns promptly. Prepare nutritious meals aligned with dietary restrictions and preferences while maintaining cleanliness during food preparation. Support patients with medication administration and ensure compliance with care plans under HIPAA regulations. Utilize patient transfer methods such as Hoyer lifts or manual handling techniques to ensure safe mobility assistance. Provide transportation for medical appointments, errands, or community activities to promote social engagement. Maintain a clean and safe environment by performing light housekeeping tasks and adhering to infection control protocols. Document care activities accurately and communicate effectively with healthcare teams, clients, and family members. Skills Proven experience working in home health, assisted living facilities, nursing homes, or group homes with a focus on geriatrics or disability support. Knowledge of patient observation techniques, dementia care—including Alzheimer's and memory care—and neurodevelopmental disorders. Familiarity with care plans, medication administration, vital signs monitoring, and patient transfer methods such as Hoyer lifts. Strong understanding of resident's rights, HIPAA compliance, and confidentiality standards. Ability to perform manual handling tasks safely while assisting with ADLs and heavy lifting when necessary. Excellent communication skills for client interaction and team collaboration within home & community care settings. Certification in CPR and first aid is required; additional training in hospice or palliative care is advantageous. Experience working with individuals with developmental disabilities or intellectual disabilities is highly valued.
