•We are seeking compassionate and dedicated Health Caregivers to provide exceptional support and personalized care to individuals across various settings, including home health, assisted living, nursing homes, and community-based programs. As a vital member of our care team, you will empower clients to maintain their independence, enhance their quality of life, and achieve their health and wellness goals. This role offers an opportunity to make a meaningful difference every day through attentive patient observation, assistance with daily activities, and compassionate support. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' vital signs and observe for any changes in health status or behavior Administer medication safely following care plans and HIPAA regulations to ensure patient confidentiality Support residents with memory care needs such as Alzheimer's and dementia by implementing specialized care strategies Prepare nutritious meals tailored to individual dietary requirements and assist with meal consumption when needed Utilize patient transfer methods such as Hoyer lifts and manual handling techniques to ensure safe mobility Maintain detailed documentation of patient care activities, observations, and progress in compliance with healthcare standards Provide personal transportation assistance for appointments or community outings as required Follow all safety protocols including first aid, CPR certification, and emergency response procedures Collaborate with healthcare teams to develop and update personalized care plans that reflect each client's needs Qualifications Prior experience working in group homes, nursing homes, assisted living facilities, or home health environments is highly preferred Demonstrated ability to perform patient observation and monitor vital signs accurately Knowledge of dementia, Alzheimer's care, memory care, and neurodevelopmental disorders is advantageous Experience working with individuals with disabilities or developmental delays is a plus Strong communication skills for effective client interaction and team collaboration Ability to perform heavy lifting and manual handling safely when assisting with transfers or mobility tasks Familiarity with medication administration procedures and care plan implementation Valid driver's license for personal transportation assistance if necessary Join our team as a Health Caregiver to provide heartfelt support that improves lives!
