We are a small but mighty home care company located in Jaffrey and surrounding towns. Looking for a care coordinator who can also help to cover clients when staff call out or on vacation. Care coordinator completes intakes of potential new clients, creating a care plan and training staff in the home to make sure needs are met. Also does quality assurance reviews every 6 months with clients and their families as needed to make sure current care plan is up to date. On site reviews with staff at clients homes every 6 months as well. Flexibility is key as call outs happen often and of course, unpredictably. We also cover hours for new clients as we shift employees schedules to match or if needed begin the hiring process for the new clients to have staff. Must have a valid drivers license and clean driving history. Must be willing to drive. We also have clients in Keene and some of it's surrounding towns. This admin will help to cover that area as well and work to expand as it becomes available. Training is available and on going. Looking for someone with a strong care giving back ground such as LNA, although does not need to currently hold that level of licensing.

Pay:

$18.00 - $20.00 per hour
