Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene to promote independence and dignity. Support patients with mobility and transfers using appropriate techniques such as Hoyer lifts and patient transfer methods to ensure safety. Monitor vital signs, observe patient conditions, and report changes promptly to healthcare professionals to maintain optimal health management. Prepare nutritious meals tailored to dietary needs, assist with medication administration following care plans, and ensure proper medication adherence. Provide companionship and emotional support while engaging clients in meaningful activities to enhance mental well-being. Maintain accurate documentation of patient care activities, vital signs, medication logs, and incident reports in compliance with HIPAA regulations. Assist with light housekeeping tasks including cleaning, laundry, and maintaining a safe living environment to promote overall health. Requirements Proven experience working as a caregiver in home health settings, assisted living facilities, nursing homes, or group homes. Knowledge of patient observation techniques, dementia care (including Alzheimer's and memory care), hospice care, and neurodevelopmental disorders such as autism. Ability to perform manual handling tasks safely, including heavy lifting and patient transfers using proper equipment like Hoyer lifts. Certification in CPR and First Aid is required; additional certifications such as HIPAA training are preferred. Strong communication skills for effective client interaction and documentation; experience working with individuals with disabilities or developmental disabilities is a plus. Familiarity with care plans, medication administration protocols, patient monitoring, and home & community care practices.
