Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient health by taking vital signs such as blood pressure, pulse, and temperature; observe for changes in condition Support medication administration following care plans and HIPAA regulations to ensure privacy and safety Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption as needed Maintain a clean and safe environment through regular cleaning duties and adhering to infection control protocols Use specialized equipment such as Hoyer lifts or resident's to safely transfer individuals with mobility challenges Document client progress, care activities, and incidents accurately in compliance with agency policies Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home & community care settings is highly preferred Knowledge of patient observation techniques, dementia care (including Alzheimer's and memory care), and working with individuals with developmental disabilities or autism Certification in CPR and First Aid is required; additional certifications such as HIPAA training or medication administration are a plus Ability to lift heavy objects safely and perform physically demanding tasks regularly Strong communication skills to effectively collaborate with team members, clients, families, and healthcare professionals Demonstrated patience, compassion, and a genuine desire to support vulnerable populations Experience with care plans, patient monitoring, hospice or home health care is advantageous Join us in providing compassionate support that empowers individuals to live their best lives.
