Duties Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods. Monitor patient health by observing vital signs, managing medication administration, and reporting any changes to healthcare professionals promptly. Support residents with memory care needs such as Alzheimer's and dementia by implementing personalized care plans and engaging them in meaningful activities. Prepare nutritious meals tailored to individual dietary requirements and assist with meal consumption when necessary. Maintain a clean and safe environment by performing light cleaning duties, ensuring infection control protocols are followed according to HIPAA standards. Provide transportation assistance for appointments or community outings, ensuring safety during transfers and travel. Document all care activities accurately in accordance with healthcare regulations and communicate effectively with clients, family members, and team members about resident progress. Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health settings is highly preferred. Demonstrated ability to observe patients closely and respond appropriately to their needs. Knowledge of patient monitoring techniques including vital signs measurement and medication management. Experience working with individuals with autism, neurodevelopmental disorders, intellectual disabilities, or developmental disabilities is advantageous. Familiarity with care procedures such as CPR, first aid, hospice care, dementia care, and Alzheimer's care is desirable. Ability to perform manual handling tasks safely, including heavy lifting and patient transfers using equipment like Hoyer lifts. Strong communication skills for effective client interaction and team collaboration. Certification in CPR and first aid is a plus; ongoing training in HIPAA compliance and home & community care practices is encouraged.
