Job Description

Overview We are seeking compassionate and dedicated In Home Caregivers to provide exceptional support and assistance to individuals in their own homes. The ideal candidate will possess a strong background in patient care, home health, and community-based support, with a focus on enhancing quality of life through personalized care. This role offers an opportunity to make a meaningful difference by delivering comprehensive care tailored to each client's unique needs, including those with developmental disabilities, neurodegenerative conditions, or requiring senior care. Applicants should demonstrate excellent communication skills, patience, and a proactive approach to caregiving. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene to promote independence and dignity. Monitor and observe patients' health status, including vital signs and behavioral changes, ensuring timely reporting to healthcare professionals. Administer medications accurately following prescribed care plans while adhering to HIPAA regulations for patient confidentiality. Support clients with mobility and transfer methods using equipment such as Hoyer lifts or patient transfer techniques to ensure safety during movement. Prepare nutritious meals tailored to dietary restrictions and preferences, supporting overall health and well-being. Provide transportation assistance for appointments, errands, or community activities to foster social engagement and independence. Maintain a clean and safe environment through light housekeeping tasks such as cleaning, laundry, and organization in accordance with infection control standards. Implement specialized care strategies for clients with Alzheimer's disease, dementia, autism spectrum disorder, or other neurodevelopmental conditions. Assist with medical procedures including first aid, CPR certification maintenance, and basic nursing tasks as required. Collaborate with healthcare teams to develop and follow individualized care plans that address specific needs related to geriatrics, hospice care, or developmental disabilities. Experience Prior experience working in home health settings or community-based caregiving environments is essential. Demonstrated proficiency in patient observation, monitoring vital signs, medication administration, and implementing care plans. Experience working with individuals with Alzheimer's disease, dementia, autism spectrum disorder, or intellectual disabilities supports effective personalized care delivery. Knowledge of manual handling techniques and patient transfer methods ensures safety during mobility assistance. Familiarity with HIPAA compliance standards guarantees confidentiality and privacy of client information. Previous roles in assisted living facilities or nursing homes provide valuable insights into long-term care practices. Ability to handle heavy lifting safely when assisting clients with mobility or transfers is required. Proven ability to communicate effectively with clients and their families while maintaining professionalism at all times.
