This position requires the CM to coordinate services with VA facilities, community referrals, and in-house resources. The CM will establish working/referral networks, while promoting a positive image of NCHP.

Preferred:

Master's Degree in Social Sciences and a minimum of 2 years of direct experience working with homeless Veterans and families. Minimum Bachelor's Degree in Social Sciences and a minimum of 3 years of direct experience working with homeless Veterans and families.
