This role is responsible for conducting assessments, developing service plans, and coordinating access to housing, healthcare, income, and supportive services. The Case Manager serves as a primary point of contact for guests, supporting their progress toward stability and permanent housing through consistent engagement, advocacy, and collaboration. This position ensures services are delivered in a trauma-informed, coordinated, and outcome-driven manner aligned with Pacific House standards and Fairfield County best practices. Essential Duties and Responsibilities Client Intake & Assessment Conduct comprehensive intakes and needs assessments Develop individualized service plans with clear goals and action steps Identify barriers to housing and stability and update plans as needed Case Management & Service Coordination Provide ongoing case management focused on housing placement and stabilization Connect guests to benefits, employment, healthcare, and behavioral health services Support navigation of public systems and track progress toward goals Collaboration & Team Coordination Collaborate with shelter staff, outreach teams, and program partners Participate in case conferencing and team meetings Coordinate care to ensure consistent and aligned service delivery Resource Development & Partnerships Maintain current knowledge of community resources and housing options Establish and sustain relationships with service providers and housing partners Facilitate referrals and follow-ups to ensure service access Documentation & Compliance Maintain accurate, timely, and complete client records in HMIS and internal systems Track service delivery, housing outcomes, and program metrics Ensure compliance with agency policies and funder requirements Crisis Intervention & Advocacy Respond to crisis situations and support guest safety and de-escalation Advocate for guest access to services and removal of systemic barriers Coordinate with internal and external partners during critical incidents Client Education & Engagement Provide coaching on life skills, budgeting, and housing readiness Encourage active participation in goal setting and service planning Promote accountability and self-sufficiency among guests Qualifications Bachelor's degree in social work, Psychology, Sociology, or related field preferred; equivalent experience considered. Knowledge of homeless services, housing systems, and community resources preferred Experience 2-4 years of experience in case management or human services Experience working with individuals experiencing homelessness or other vulnerable populations Familiarity with trauma-informed care and service planning practices
