Saint Petersburg, FL Job Details Part-time $20 an hour 2 hours ago Qualifications Care plan evaluation Program design Patient advocacy Mid-level CAC Client advocacy in social work Patient progress monitoring 2 years Direct patient care in social services case management Client progress monitoring in health services social work Full Job Description Manages and monitors assigned caseload of Beneficiaries participating in a rehabilitation program; develops program plan/goals and evaluates beneficiaries progress by conducting routine meetings with Beneficiaries and/or staff; serves as advocate for Beneficiaries in order to acquire services that will enable them to functionally cope with their environment. Education and Experience A minimum of two (2) years of satisfactory employment experience working in direct case management, social services or related field. or any equivalent combination of training and experience which provides the required knowledge, skills, and abilities. Licenses and Certifications Certified Addiction Counselor (Certificates) Preferred but not required.
