This role provides housing assessments, resource navigation, financial assistance support, and coordination with community partners and internal programs. Key Responsibilities
  • Assess client housing needs and develop individualized Housing Care Plans.
  • Connect clients with affordable housing programs, financial assistance, and community resources.
  • Assist clients with housing applications, required documentation, and financial assistance programs.
  • Verify client information and financial documentation for housing assistance applications.
  • Collaborate with Case Managers, Linkage Navigators, and other departments to address client housing needs.
  • Communicate with clients, landlords, utility providers, medical providers, and community agencies to coordinate services.
  • Connect clients to additional resources such as healthcare, employment, job training, and skills development.
  • Maintain accurate client records, documentation, tracking, and required reports.
  • Ensure compliance with program requirements and applicable guidelines.
  • Provide respectful, client-centered services to individuals from diverse backgrounds.
  • Perform other related duties as assigned.
Qualifications Bachelor's degree in Human Services, Social Services, or a related field. Minimum 2 years of experience in social services/housing programs, such as Section 8, HOPWA, or HUD. Experience working with individuals living with HIV, LGBTQ+ communities, communities of color, substance use populations, or other high-risk populations strongly preferred. Knowledge of HIV/AIDS and community resources. Strong communication, organization, documentation, and customer service skills. Proficiency with Microsoft Office. Ability to prioritize, multitask, work independently, and collaborate effectively in a fast-paced environment.
