This role provides housing assessments, resource navigation, financial assistance support, and coordination with community partners and internal programs. Key Responsibilities
- Assess client housing needs and develop individualized Housing Care Plans.
- Connect clients with affordable housing programs, financial assistance, and community resources.
- Assist clients with housing applications, required documentation, and financial assistance programs.
- Verify client information and financial documentation for housing assistance applications.
- Collaborate with Case Managers, Linkage Navigators, and other departments to address client housing needs.
- Communicate with clients, landlords, utility providers, medical providers, and community agencies to coordinate services.
- Connect clients to additional resources such as healthcare, employment, job training, and skills development.
- Maintain accurate client records, documentation, tracking, and required reports.
- Ensure compliance with program requirements and applicable guidelines.
- Provide respectful, client-centered services to individuals from diverse backgrounds.
- Perform other related duties as assigned.
