  • Conduct comprehensive assessments of clients' needs and develop individualized care plans
  • Provide case management services to clients, including coordinating and monitoring their access to necessary resources and services
  • Collaborate with other professionals, such as social workers, counselors, and healthcare providers, to ensure holistic care for clients
  • Advocate for clients' rights and assist them in navigating complex systems and processes
  • Monitor clients' progress towards their goals and make necessary adjustments to their care plans
  • Document all client interactions, services provided, and progress made in case management software or databases ```Experience```
  • Bachelor's or Master's degree in social work or a related field
  • Previous experience in case management or a similar role preferred
  • Knowledge of social work principles and practices, crisis intervention techniques, child welfare systems, behavioral health services, addiction counseling approaches, motivational interviewing techniques, workforce development programs, Section 8 housing assistance, supervising staff members, and Medicare regulations
  • Strong communication skills with the ability to effectively interact with diverse populations
  • Excellent organizational skills and attention to detail
  • Ability to work independently and as part of a team
