Senior Services Case Manager
QUALIFICATIONS 1.
Bachelor's Degree in the area of human services or related field required. 2. Four to Six years of work experience in Human Services is required. 3. Supervisory experience required. 4. Must possess an awareness of, and sensitivity to, challenges facing low-income, elderly and handicapped individuals and willingness to work with persons struggling to achieve/maintain self-sufficiency. 5. Knowledge of community resources available within area of responsibility preferred. 6. Must have ability to work independently with minimal supervision and have strong interpersonal communication skills. 7. Must maintain a valid driver's license, reliable transportation, and ability to travel within the service area. 8. Must successfully pass all agency-required background checks. 9. Must be sympathetic to the goals of a Community Action Agency. WAGE $27.00/hour plus agency fringe benefits Fringe benefits include, a four-day work week, health insurance, life insurance, paid time off, 11 paid holidays per year.

RESPONSIBILITIES 1.
Provide immediate supervision to assigned staff. 2. Responsible for oversight of assigned services which include, but are not limited to, customer caseloads and home visiting schedules, development of customer service plans, ensuring production levels of Outreach, Case Coordination and Support, Caregiver Case Management, and Home Delivered Meal services. 3. Continuously monitor Case Coordination and Support, Caregiver Case Management, Caregiver Training, and Outreach services to ensure customers are provided with effective services and that contract goals are met. 4. Provide initial training for new staff and ensure a team-based approach in providing senior services. 5. Responsible for conducting complete home-based comprehensive assessments of individuals over 60 years of age, develop a person-centered plan, provide internal and external referrals, complete phone monitoring calls.
