Case Manager

Job Description

.Position Summary Reveal 2 Heal, LLC is seeking a compassionate, dependable, and resourceful Case Manager to join our team. The Case Manager will provide care coordination and advocacy services to individuals and families by connecting them with essential community resources that support their mental, physical, and social well-being. The ideal candidate is passionate about empowering others, has strong organizational skills, and is committed to helping clients overcome barriers to achieving stability and independence. Essential Duties and Responsibilities Conduct comprehensive client assessments to identify needs, strengths, and barriers. Develop individualized service plans based on each client's goals and needs. Assist clients in securing safe and affordable housing. Connect clients to mental health counseling, therapy, psychiatric services, and other behavioral health resources. Assist clients with obtaining or maintaining health insurance, including Medicaid and other available programs. Help clients apply for Social Security benefits (SSI/SSDI), SNAP, TANF, and other public assistance programs. Coordinate referrals to medical providers, employment services, educational programs, transportation resources, food assistance, and additional community supports. Advocate for clients with landlords, healthcare providers, government agencies, and community organizations. Monitor client progress and update service plans as needed. Maintain accurate, timely, and confidential documentation in accordance with agency policies and applicable regulations. Participate in case conferences, team meetings, and ongoing professional development. Build and maintain strong relationships with community partners and service providers. Minimum Qualifications Associate's Degree required in Human Services, Social Work, Psychology, Sociology, Counseling, or a closely related field. Bachelor's Degree preferred in Social Work, Human Services, Psychology, Counseling, or a related discipline. At least one (1) year of experience in case management, social services, behavioral health, or a related field is preferred. Knowledge of community resources, housing programs, public benefits, and healthcare systems is highly desirable. Strong verbal and written communication skills. Excellent organizational, problem-solving, and time management abilities. Ability to maintain confidentiality and professional boundaries. Proficiency in Microsoft Office and electronic documentation systems. Valid driver's license and reliable transportation are preferred. Must be able to work 8:30am-3:30pm or 9:00am-4:00pm Preferred Qualifications Experience working with Medicaid populations. Knowledge of housing navigation and supportive housing programs. Familiarity with SSI/SSDI, SNAP, TANF, and other public benefit programs. Experience collaborating with behavioral health providers and community agencies. Ability to work independently while managing multiple client cases. Bilingual candidates are encouraged to apply.
