As the Harm Reduction Case Manager , you will play a pivotal role in individuals' lives, making a significant impact through: Conduct outreach activities to engage the target population Identify community partners to engage PWUD who are unaware of their status or untreated Implement harm reduction strategies aimed at helping individuals minimize their immediate health and safety risks Provide enrollment into the agencies Syringe Exchange Program Facilitate brief assessments with the target populations Through review of the assessment, identify community based services to provide client linkage Facilitate linkage to services through escort as necessary

Mon - Fri 8:30am - 4:30pm

Qualifications:
