Job Description
Job Overview We are seeking a dedicated and compassionate Case Manager to join our dynamic team, committed to empowering individuals and families through personalized support and resource coordination. In this vital role, you will serve as a guiding partner, helping clients navigate complex social systems, access essential services, and achieve their goals. Duties Assess clients' needs through detailed interviews and comprehensive evaluations, identifying immediate risks and long-term goals. Provide crisis intervention and crisis management support during urgent situations, ensuring client safety and stability. Utilize motivational interviewing techniques to motivate clients toward positive behavioral changes and sustained engagement with services. Offer addiction counseling support when appropriate, guiding clients through recovery pathways and relapse prevention strategies. Maintain thorough documentation of all client interactions, service plans, progress notes, and incident reports in compliance with organizational policies. Collaborate with community agencies, child welfare organizations, and housing authorities to ensure seamless service delivery. Skills Strong crisis intervention skills with the ability to remain calm under pressure and manage urgent situations effectively. Expertise in addiction counseling techniques coupled with a solid understanding of behavioral health principles. Proven experience in crisis management and conflict resolution within social service settings. Deep knowledge of social work practices, including client advocacy and case planning. Proficiency in motivational interviewing to foster client motivation and engagement. Knowledge of workforce development initiatives aimed at employment readiness and skill-building for diverse populations.
