Job Description

Protective Services Care Manager for Older Adults Work life balance is very important at the Office of Human Services! The Older Adult Protective Services Care Manager plays a crucial role in safeguarding older adults from abuse, neglect, exploitation, and abandonment. Here's a brief overview of the job description:
Key Responsibilities:
Investigations:
Receive and investigate reports of elder abuse, neglect, exploitation, and abandonment.
Assessment:
Evaluate the environmental, medical, social, financial, and psychological needs of older adults to determine if abuse, exploitation, or neglect is substantiated.
Case Management:
Develop and implement care plans for older adults, collaborating with family members, caregivers, friends, service providers, and community resources to reduce risk and support independence. The average case load is between 5-10 consumers.
Emergency Interventions:
Assist in emergency interventions such as initiating emergency medical care, mental health commitments, or protection from abuse orders.
Documentation:
Document all case activities in accordance with state regulations and agency policies.
Coordination:
Work closely with inner office home and community based programs staff, public service providers, mental health professionals, law enforcement, and EMS, financial institutions, and other community organizations to coordinate care.
Work Environment:
Dedicated base office space with flexible hybrid work form home model after successful completion of probationary period (2 days work from home; 3 days in office) to ensure a positive work/life balance Travel throughout Cameron, Elk, McKean Counties (mileage reimbursed at highest IRS allowable rate)
Salary Range :
$22.00 - $25.00 per hour + on-call stipend, mileage reimbursement at highest allowable tax- exempt IRS rate
Status:
Full Time-Non-exempt Work Hours:
Monday through Friday 8:00am- 4:00pm + on-call every 6-10 weeks (hourly on-call stipend plus premium pay) Applicant must:
  • Have strong time management skills and cope well under stressful situations
  • Be a strong consumer advocate who can maintain a balance of objectivity and empathy
  • Have excellent communication skills and quick decision making abilities
  • Enjoy working in a team environment providing support to others
  • Have advanced computer and documentation skills
  • Have a reliable vehicle for work related travel, auto insurance and a valid driver license
  • Pass a criminal background check, child abuse clearance and possible FBI clearance
  • Pass pre-employment drug and alcohol test
  • Must be available to cover on-call hours when required (approx.
6x per year- paid at a premium rate plus hourly stipend) This is a full-time 37.50 hours per week position, working 8:00am - 4:00pm with .5 unpaid lunch break. Participation in after hours on-call rotation every 6-10 weeks is required.
Minimum education and experience:
Bachelor's degree in Social Services, Criminal Justice, Nursing, Human Services or a related field AND
  • 1 year of experience in mandated reporter incident investigation, assessment, care management, social work, home health, health care, or related environment. OR Associate's degree in Social Services, Criminal Justice, Nursing, Human Services or a related field AND
  • a minimum of 2 years experience in mandated reporter incident investigation, assessment, care management, social work, home health, health care, or related environment.
