JOB SUMMARY
The Family Gateway Rapid Rehousing Housing Stability Case Manager - Domestic Violence is responsible for the overall delivery of trauma-informed, client-centered services and implementation of individualized case plans for families participating in the Rapid Rehousing - Domestic Violence Program. The Housing Stability Case Manager provides intensive case management, crisis intervention, housing navigation, housing stabilization, harm reduction, and supportive services to assist survivors and their families in obtaining and maintaining safe, stable, permanent housing. The position works closely with families, landlords, property management companies, community partners, and internal Family Gateway staff to reduce barriers to housing and promote long-term housing stability and self-sufficiency. The Housing Stability Case Manager is responsible for ensuring timely documentation, housing inspections, move-in coordination, financial assistance requests, and compliance with program and funding requirements.
JOB DUTIES/RESPONSIBILITIES
Provide intensive, trauma-informed case management services to approximately 25 families , with an emphasis on the needs and goals of adult household members. Conduct Rapid Rehousing program orientations and explain program policies, expectations, participant responsibilities, and available services. Provide housing navigation and assist families with housing searches, applications, landlord communication, lease signing, and move-in coordination. Conduct required housing inspections to ensure units meet applicable program, habitability, and funding requirements before move-in. Coordinate and provide move-in kits and essential household items to assist families with establishing a safe and stable home. Submit and track payment requests through Padmission for eligible housing-related financial assistance, including rental assistance, security deposits, utility assistance, and other approved program expenses. Assist with the collection, review, and submission of required documentation to support timely payment processing. Cultivate professional and trusting relationships with families while maintaining appropriate boundaries and confidentiality. Provide services using trauma-informed, survivor-centered, culturally responsive, and harm-reduction approaches . Address participant needs and develop individualized case plans focused on housing stability, safety, income, employment, and long-term self-sufficiency. Provide crisis intervention and supportive counseling within the scope of the position and make appropriate referrals to domestic violence, behavioral health, medical, legal, employment, and other community resources. Conduct monthly home visits to assess housing stability, discuss progress toward case plan goals, identify emerging concerns, and provide ongoing support. Conduct quarterly assessments of participant progress and update individualized service plans as needed. Correctly calculate participant rent contributions using the applicable declining subsidy model and participant ability to pay. Monitor housing stability and intervene proactively when barriers, lease concerns, landlord issues, or other risks to housing retention arise. Facilitate positive relationships and communication between landlords/property managers and participating families. Assist participants with applying for and obtaining long-term sources of income, including employment, mainstream benefits, and other eligible resources. Provide appropriate resources, tools, referrals, and supportive services to assist participants in achieving case plan goals; follow up on referrals to support successful connections to services. Maintain accurate and timely participant records and document all services, contacts, housing developments, inspections, and case activity in HMIS within 48 hours of the participant contact or case development. Maintain accurate documentation in Padmission and other required program systems. Complete required program reports, payment documentation, housing documentation, and other administrative requirements accurately and within established deadlines. Monitor participant eligibility, program compliance, and documentation requirements throughout the participant's enrollment. Ensure program services are delivered in accordance with applicable Federal, State, Continuum of Care (CoC), Family Gateway, and other funding-source requirements . Maintain confidentiality and follow all applicable policies and procedures related to domestic violence survivors, personally identifiable information, and sensitive client information. Support participant safety planning and ensure services are provided in a manner that prioritizes survivor choice, confidentiality, autonomy, and safety. Work collaboratively with Family Gateway's Coordinated Access/Assessment, Diversion, shelter, case management, and other program teams as appropriate. Maintain effective communication with community partners, domestic violence service providers, landlords, property managers, and other stakeholders. Participate in staff meetings, case consultations, Family Gateway meetings, and outside agency meetings relevant to the position. Attend a minimum of four trainings annually to increase knowledge and skills related to domestic violence, trauma-informed care, housing stabilization, Rapid Rehousing, and effectively meeting participant needs. Maintain a work schedule responsive to program and participant needs, including flexibility for home visits, housing inspections, landlord meetings, and other required activities. Ensure program performance goals are met, including the established goals for housing placement/retention and participant income growth. Ensure 87% of participating families transition to and/or maintain permanent housing , consistent with applicable program performance standards. Ensure 55% of participating families gain earned income and/or increase household income while enrolled in the Family Gateway Program. Perform all other duties as assigned.
QUALIFICATIONS
Bachelor's degree in social work, Human Services, Psychology, Counseling, or a related field required. Minimum of two years of experience working with diverse populations, with experience serving survivors of domestic violence and families experiencing homelessness preferred. Prior Rapid Rehousing experience required . Prior experience with HMIS and Padmission Journey preferred . Knowledge of housing navigation, housing stabilization, homelessness services, domestic violence services, and community resources. Experience conducting housing inspections or willingness and ability to become proficient in program-required inspection procedures. Experience processing or coordinating rental assistance, security deposits, move-in expenses, or other housing-related financial assistance preferred. Ability to learn and effectively utilize Padmission for submitting and tracking payment requests. Ability to provide crisis intervention, problem-solving, case management, resource coordination, and supportive services. Knowledge of trauma-informed care, harm reduction, and survivor-centered practices. Strong written and verbal communication skills. Strong organizational, documentation, time-management, and problem-solving skills. Ability to maintain confidentiality and exercise sound judgment when working with sensitive domestic violence and housing-related information. Ability to work independently while also functioning effectively as part of a multidisciplinary team. Ability to establish and maintain professional relationships with participants, landlords, property managers, community partners, and internal staff. Must have reliable transportation and the ability to travel to participants' homes, housing sites, property management offices, and community locations. Must be able to conduct home visits and housing inspections safely and professionally.
WORKING CONDITIONS
This is a full-time position subject to the Fair Labor Standards Act. The standard schedule is Monday through Friday, with flexibility based on program and participant needs. The position requires regular travel to participant homes, housing properties, community agencies, and other locations. Occasional evening or schedule adjustments may be required to accommodate participant needs, housing inspections, landlord meetings, and program activities. The Housing Stability Case Manager must be able to work independently, manage multiple cases and competing priorities, meet documentation and payment-processing deadlines, and work collaboratively within a team environment. This is a grant position until November 2027; grant cycle may be subject for renewal.
